The GPs at Elwood Family Clinic (EFC) would appreciate it if you could complete a survey.

The survey is being conducted by the University of Melbourne.

The survey is about your experience of using telehealth, either in your recent consultation with one of the EFC GPs, or in your upcoming consultation with one of the EFC GPs.

Completing this survey will help general practice to improve phone or video call consultations in the future.
